APS Travel Concierges offers you full assistance in reviewing and selecting extraordinary

SENIOR ADVOCACY SERVICES AND POWER OF ATTORNEY

APS is s central point of contact to services and programs available for senior citizens of Clark County to ease the frustrations in accessing assistance when needed

Provides information and referral on several topics including:
*Medicare/Medicaid
*Prescription drug programs
*Utility assistance
*Supplemental insurance
*Legal assistance
*Senior housing - Group Homes, Nursing Homes, Senior etc.
*Tax rebates
*Social service programs
*VA Assistance/Champs/Programs
*Moving & relocation

APS Provides Assistance:
Provides rent and utility assistance to seniors age 62 and over
Provides utility assistance through Project Reach to seniors age 62 or over and disabled individuals who are medically fragile

Homemaker Home Health Aid assistance:
The Homemaker Home Health Aide Program provides short and long term homemaking and personal care services to individuals, with special attention given to the needs of low-income, elderly/disabled individuals/Veterans.

Additional Services include:
Grocery shopping
Laundry
Light housekeeping
Food preparation
Personal Care (baths and grooming)
RX pick-up

Target Population/Area:
Seniors and residents of Clark County, who demonstrate a need for service, and are not eligible for other programs. Can refer to all needed resources to fit your unique personal situation.
Eligibility Requirements:

Senior or resident of Clark County
Not eligible for similar services through insurance
or another program/can assist if need additional help with services or referrals
Disabled and/or have a medical need for services including Veterans

APS offers service to prepare and provide paperwork for income and asset information given to Clark County Social Service and government agencies as needed to determine eligibility. This includes other entities of government for care such as Medicaid/Medicare/Home Aid/Pharmacy/Rehab/VA etc.


Long Term Care:

APS can assist in securing appropriate long term care for anyone who is unable to function in independent living because of medical problems. Eligibility for long term care is determined by an assessment of need and pre-established eligibility criteria. All prior resource programs to assist with payment of long term care are pursued.

APS to submit and/or request for LTC services are received from the community, (including staff from long term care facilities and hospitals), Clark County Social Service staff, and other agencies that have knowledge of potential long term care cases, families of persons in need of this care, or clients applying on their own behalf.

The types of licensed facilities contracted to provide care in Las Vegas are:

1.
Adult Day Health Care Facility
These facilities provide organized day programs of therapeutic, health, and social activities and services for chronically ill, elderly or disabled adults for restoring individuals to his or her best possible functional level.

2.
Adult Group Care and Assisted Living Facilities
These are residential care facilities. They are ideal for persons who, because of age or disability, would not be able to provide their own household needs and need someone in attendance in case of emergencies.

3.
Nursing Homes
Free Standing Intermediate Care Facility
These facilities provide custodial care and supervision under the direction of a professional registered nurse.
